GRANT APPLICATION PROCESS


Grant applications must be submitted by the US Postal Service First Class Mail to the address listed below:

Applications may not be submitted in any other manner than as mentioned above.  Do not use staples, notebooks, or folders.  Do not bind any applications.  Do not provide any other supporting materials. Do not submit a two-sided application.  Applicants may, but are not required to, submit a one or two page cover letter with their application.

This application form must be completed and submitted by all applicants.

The Fern and Gladys Moyer Memorial Trust typically meets on a quarterly basis.  Applications will be reviewed at a Fern and Gladys Memorial Trust meeting and a determination will be made to fund some or all of the request, deny the request, or decide to ask for additional information.

If all requested information is provided prior to the next meeting, then the request will be decided upon at the next meeting.

The Fern and Gladys Moyer Memorial Trust will notify applicants of any and all actions taken regarding their applications.

WHO IS ELIGIBLE TO RECEIVE GRANTS?


501(c)(3) IRS Approved Non-Profit Organizations, County Agencies and Municipal Agencies that serve the youth of the Pennsylvanian Counties of Northumberland and Snyder.

Grant applications must be for projects and programs that help the youth of the Pennsylvanian Counties of Northumberland and Snyder.

“Youth” typically refers to people under the age of 18.  “Youth” also includes people who due to an intellectual disability have an intellectual level comparable to someone under the age of 18.  The polite terminology for people with intellectual disabilities may vary from time to time.  “Intellectual disability” is currently considered to be a polite way to refer to people whose youth is determined by a means other than their age.  If this term subsequently is determined to be impolite, please understand that no disrespect is intended.

Religious and political projects and programs are ineligible for funding even if the intent of those projects and programs is to help the youth of the Pennsylvanian Counties of Northumberland and Snyder.

FINAL AND INTERIM STATUS REPORT REQUIREMENTS FOR GRANT RECIPIENTS


All grant recipients organization must provide at least one status report to the Trust for each grant that your organization received.  Status reports are either final status reports or interim status reports. A final status report is due when the project is completed.  An interim status report is due every six (6) months from when the grant money was paid to your organization. The status report form is always mailed to the organization along with the check from the Trust. If the status report form is lost or misplaced, then submit the information in the checklist below.

Failure to comply with status report requirements may make your organization ineligible to receive additional grant money.

Every status report shall include the following information in the checklist below:

1. State whether the status report is a final status report or an interim status report.
2. Name, address and EIN of organization
3. Name and telephone number of contact person associated with the program or project.
4. Program or project name, date grant was approved (month & year) and amount awarded.
5. How was the money from the Trust spent for the program or project?
6. Must provide copies of all receipts and expenses pertaining to the program or project. Failure to do so may affect future grant requests.
7. If applicable, a refund of any portion of unused grant proceeds.

For interim status reports, in addition to providing the above information, please also provide a report on whether your organization believes that the project will be completed within six months from the date of the submitted status report and your reasonings in support thereof.

REVIEW OF APPLICANT’S REQUEST

The Fern and Gladys Moyer Memorial Trust will review submitted applications and will decide to fund the request, deny the request, or ask that additional information be provided. For those funding requests requiring additional information, the applicant may be contacted to submit the following:

  • Further clarification of project goals and objectives, time-frame for completion.
  • Organizational overview of mission, history and services provided.
  • Further clarification of budget items, anticipated project expenditures and other sources of support for the project.
  • A statement describing future funding for sustaining the project in future years.

NOTIFICATION TO APPLICANTS

The Fern and Gladys Moyer Memorial Trust will notify applicants of its final decision of all grant requests.

The region served by the Fern and Gladys Memorial Trust is restricted to Northumberland and Snyder Counties, Pennsylvania.
